EXERCISE RECOMMENDATIONS FOR THE ELDERLY

Exercise Recommendations For The Elderly.

     Geriatrics is a branch of medicine that deals with the problems and diseases associated with elderly people above 65 years and the aging process. As people age, they go through physiological process that are natural and likely enhanced by certain conditions ( example, inactivity, environmental factors).For the purpose of description, the elderly are divided into the old age(65-74 years of age),the very old age (75-84 years of age),and the oldest old (older than 85 years of age).Although these groups chronologically categorize the elderly, they are a heterogeneous group of individuals in terms of physiological status and there is overlap between groups.

      Medical History

Mrs X is a 75 -year-old black  woman who has a history of falling and osteoarthritis in her hands. She has been a House wife for most of her life and has never performed any type of regular physical activity. She lives by herself and has smoked half a pack of cigarettes a day for the past 35 years. Her body mass index is 30,her blood pressure is 142/78mmHg.

           Diagnosis

Mrs X went to see her doctor with complaints about her inability to complete normal daily living tasks such as carrying bags, walking around the house without worry of falling, and going up and down her basement stairs. She was unable to stand from a chair  of standard height without using arms and unable to step up onto a 23-cm step. She also indicated that she feels lonely during most  days of the week. A resting electrocardiogram showed no abnormalities. No significant diagnostic findings were made. The physician recommended she begin an exercise routine and ordered an exercise test.

          Exercise Test Results

The patient underwent an exercise test. Her electrocardiogram demonstrated isolated premature ventricular contractions and normal ST segments at peak exercise. The following values were noted:

   * Resting heart rate-82

*Resting blood pressure-137/80

*Peak heart rate-144

*Peak blood pressure-221/92

*Estimated Vo2 peak-14ml-kg.min


Development of Exercise Prescription

     Mrs X wishes to become stronger so she is able to perform general ADLs and remain independent. In discussion with her physician, she was referred to join the elderly adult fitness programme. She went  reluctantly, and after consultation with the exercise professional on staff she set a goal of becoming more active on a regular basis to maintain her independent living status.
